If you have been treated for Covid19 at home and would like to be issued with a "Certificate for Medical Care at Home”, please apply to the Shinagawa Health Center.

As a basic rule, the certificate that will be issued and sent out will be in the style designated by Shinagawa Health Center.

If you require a “Certificate for Medical Care at a Hotel”, call the Subsection in charge of Certificates for Medical Care at Designated Facilities at the Tokyo Metropolitan Government’s Business Promotion Section.

If you need proof of hospitalization, please contact the medical institution at which you were hospitalized, directly.

新型コロナウイルス感染症で自宅療養をされた方のうち、「自宅療養証明書」の発行を希望する方は、品川区保健所へ申請してください。

原則、証明書は、品川区保健所の指定様式にて、発行して送付いたします。

また、ホテル施設の療養証明書が必要な場合は、東京都事業推進課　宿泊療養証明担当　電話番号 03-5320-4478 までお問い合わせください。

入院の証明が必要な場合は、入院医療機関へ直接お問い合わせをお願いします。
